Exteded Care and Rehabilitation
Extended Care and Rehabilitation holds a purpose and a promise…
At Trace Extended Care and Rehabilitation Center (formerly Floy Dyer Care & Rehabilitation Center), our priorities are simple – we’re dedicated to the highest quality of care for the highest quality of life for each and every resident. Whether you’re with us for a short rehabilitative stay or making a life transition, our clinical care team delivers exceptional care from the moment you arrive.
Our medical staff provides an outstanding level of clinical competence, coupled with true compassion. Working in collaboration with personal physicians and resident families, we craft a personalized plan of care that helps facilitate the healing process and achieve the highest quality of life. Also, it helps to know that if any event arises in needing acute health services, Trace Extended Care and Rehabilitation is attached to a full-service hospital, Trace Regional.
For over 30 years, Extended Care and Rehabilitation has been committed to outstanding short- and long-term care with a warm, patient-family focus. It’s no wonder generations have trusted us for their care!
